Our Services can include:

• A Caregiver Consultation in your own home. The RBACC practitioner will go to the home to meet, assess and discuss the specific needs and issues of caregiving with the entire family, including the individual receiving the care, to determine challenges and problem areas.

After the initial consultation, RBACC will provide a list of recommendations, options, and therapeutic services that will address the needs of everyone. The goal is to provide solutions that will enhance the quality of life for the caregiver and individual.

• An evaluation by a physical and/or occupational therapist to determine need for direct therapy to:
  • improve and/or restore physical function due to illness, injury, aging or with conditions such as arthritis,multiple sclerosis (MS), Parkinson’s, Congestive Heart Failure, and stroke
  • work with the primary healthcare provider to develop solutions and implement the appropriate therapies and regimens for the individual
  • ease caregiver burdens and stress
• An evaluation through The Balance and Mobility Clinic of Columbus. Don’t wait for a fall to happen – a balance assessment can help determine an individual’s risk for falls. Many falls happen because of a weak sensory area(s) within the body, which can be identified and “treated” through specific exercises and therapies.Test results are computerized and will indicate appropriate recommendations to increase balance ability and decrease risk for falls.

• An evaluation by Home Modifications for Independence:
  • An occupational therapist (OT) will conduct an on-site assessment to address and resolve problem areas inside and outside the home
  • make the home safer so the individual can continue to perform daily routines and self-care, and enable ease of daily living through recommendations for devices, equipment and modifications specific to each individual and caregiver’s needs and home setup.
  • ease caregiver burdens and stress
A recommendation to another healthcare practitioner as appropriate to:
  • identify and treat other suspected health issues that may be contributing to or worsening an illness or condition for individual and/or caregiver
  • provide physician referral for needed occupational and/or physical therapy services
  • ease caregiver burdens and stress
A referral to private, local, state and national assistance programs and resources to:
  • determine funding sources that the family may be eligible for and how to tap into them, and to determine eligibility
  • determine eligibility for home meal delivery, aging or social service programs and education/information resources that provide direct services to the elderly, infirm, family and caregivers
  • ease caregiver burdens and stress
